This achievement is earned on the Xbox exclusive mission "Jamais Vu". To earn this, you will need to make your way through the mission without alerting any enemies, and sounding the alarm. What you'll want to do is make your way through the mission neutralizing any normal guards with your pistol, and killing any of the special guards.

To make sure that you aren't spotted, take your time around corners, and spot out enemies with your binoculars as much as possible. Also, make sure to turn off the light on your assault rifle so that you are harder to see in the shadows.

During the final part of the mission, use the RPG to kill enemies in groups, and stay behind cover so that you are not noticed. Once all enemies are taken out, the helicopter will pick you up and the achievement will unlock if you completed the mission without alerts.
