Masseter Botox for TMJD and Facial Slimming!
Over 10 million Americans suffer from temporomandibular joint disorder ( #TMJD ). It causes tooth erosion and bite problems, pain, and inflammation due to chronic clenching, grinding, and bruxism.

The typical way most aesthetic and medical providers treat the masseter is through a 3 point injection technique. This 3 point technique does not actually treat TMJD because the upper pole of the masseter is left completely untreated.

Furthermore, because a high number of units are placed in very few locations, the risk of complications is very high including smile asymmetry (from inadvertent treatment of the risorius), paradoxical bulging, and a “donut hole” effect where you have atrophy in the treated area but hypertrophy elsewhere (which can actually worsen TMJD).

Our approach of globally treating the masseter from its insertion points, from zygoma to mandible and antegonial notch to ramus, effectively treats TMJD, causes global slimming of the masseter (if desired— this can be modulated to minimize the slimming effect if patients don’t want to be slimmed), and decreases the risk of complications since each injection point only gets 1-2 units of Botox in each location.

Therefore, even if you inadvertently hit the risorius it will most likely be subclinical in its effect and thereby avoids smile asymmetry. I use a 2 unit per 0.1 cc dilution and stay at least 1 cm away from anterior border of the masseter since that is the most likely insertion point of the risorius.
